Medical Weight Management
Medical Weight Management
Physician-Directed Medical Weight Management
Designed as Medical Care, Not Another Program
You have done this before. You know how to eat well, you have followed the plans, you have had stretches where it worked. And then it stopped working, or life happened, and the weight found its way back, and the next attempt was harder than the last one.
That pattern is not a character flaw. Body weight is regulated by a system involving hormones, metabolism, appetite signaling, sleep, stress, medication effects, and medical conditions, and that system defends itself against change. Willpower is asked to overcome biology, and biology is patient.
Medical weight management treats weight as a medical concern—not simply another program to follow. At Revive Clinic & IV Therapy, care begins with a thorough evaluation and laboratory testing, followed by a personalized plan built around your unique needs. Ongoing clinical follow-up ensures your care evolves with you every step of the way.
Our Middleburg office serves patients from across western Loudoun County and the surrounding hunt country, including Aldie, Upperville, The Plains, Philomont, Purcellville, Marshall, and Leesburg. Revive is a fee-for-service practice offering both clinical care and aesthetic medicine, so your care is directed by a provider who takes your broader medical history into account.
Your first appointment is simply a conversation. Your provider will listen to your history, review what you have tried, and explain what evaluation would tell you. You will leave with a clear picture of what the program involves and no pressure to commit to anything.

What Is Medical Weight Management?
Medical weight management is weight care delivered as clinical care. It involves evaluation by a provider, laboratory testing, an individualized plan, and scheduled follow-up, with adjustments made on the basis of how you are actually doing rather than on a fixed schedule.
The distinction from a commercial weight loss program matters. A program sells you a method. Medical care evaluates you first, which means identifying whether something measurable is making weight loss harder. Thyroid function, insulin resistance, hormonal changes, sleep apnea, iron and vitamin D status, and the effects of medications you may already be taking all influence body weight, and several of them are correctable. Finding one changes the plan.
Evaluation also covers your history honestly, including previous attempts, what worked and for how long, and your relationship with food and eating. That conversation matters clinically, because it shapes what approach is appropriate and whether medication has a role at all.
Care here addresses the whole picture rather than one lever. Nutrition, protein intake, resistance training, sleep, stress, medical contributors, and where appropriate medication all form part of the plan, because weight that comes off through any single mechanism alone tends not to stay off.
The goal is not a number by a date. It is sustainable change, supported clinically, with a plan for holding it.
About Medication
Whether medication is appropriate for you, and which option fits, is a clinical decision made after evaluation. For that reason Revive does not publish a list of medications or discuss specific prescriptions before you have been evaluated.
What can be said upfront is how it is handled here.
Legitimate sourcing.
Any medication prescribed is obtained through legitimate, properly regulated channels.
Individualized dosing.
Dosing is determined for you and adjusted based on your response and tolerance rather than following a standard escalation.
Side effects reviewed before you start.
You will have a full conversation about what to expect, what is common, what is serious, and what to do about it, before anything is prescribed.
Monitoring built into the plan.
Follow-up appointments and repeat testing are part of the program rather than optional extras, and they continue for as long as treatment does.
Medication is one possible component of a plan and not the plan itself. For some patients it is appropriate and effective. For others it is not indicated, and your provider will tell you so.
What Brings People In?
- Weight that has resisted repeated, genuine effort
- Regain after previous weight loss
- Weight changes that began with a life stage, a medication, or a medical condition
- Difficulty losing weight despite consistent nutrition and training
- Weight affecting joints, sleep, energy, or mobility
- A medical reason to address weight, discussed with another physician
- Wanting evaluation rather than another program
- Wanting clinical support rather than doing it alone again
What Evaluation Covers
- Full medical history Your health conditions, surgeries, medications and supplements, and family history.
- Weight history When changes occurred, what you have tried, what worked, and what happened afterward.
- Eating and activity history Discussed clinically and without judgment, because it shapes what approach is appropriate.
- Laboratory testing Thyroid function, blood glucose and hemoglobin A1c, insulin, lipids, liver and kidney function, blood counts, iron studies, vitamin D, and hormone markers where indicated.
- Contributing factors Sleep quality, stress, and medications known to affect weight.
- Body composition Measured where available, since preserving muscle matters as much as the number on the scale
Finding a correctable contributor is a good outcome. It means part of the difficulty has an explanation and something specific to address.
Benefits of a Medically Directed Approach
- Evaluation and laboratory testing before any plan is made
- Identification of medical contributors that make weight loss harder

- Attention to preserving muscle, not only reducing weight
- Ongoing clinical follow-up and adjustment
- A provider who can coordinate with your other physicians
- A maintenance plan built in from the start rather than added at the end
- An honest assessment, including when medication is not indicated
What Patients Often Report
- Steady, sustainable progress rather than a rapid cycle
- Better energy through the day
- Improved sleep
- Clothing fitting more comfortably
- Improvements in markers their physician has been watching
- Easier movement and less joint discomfort
- Feeling supported rather than alone with it
Results vary considerably, and progress is rarely linear. Starting point, medical conditions, medications, hormones, age, sleep, stress, nutrition, and activity all affect what happens and how quickly. Periods where progress slows or pauses are a normal part of the process rather than a sign of failure. Your provider will set realistic expectations for your particular situation and adjust the plan as you go rather than asking you to try harder.
What the Process Looks Like
- Initial consultation
A full conversation about your health, your weight history, what you have tried, and what you want.
- Laboratory testing
Comprehensive baseline testing to identify medical contributors and establish where you are starting from.
- Results and planning
A dedicated appointment to review your results and build a plan, including nutrition, protein and resistance training, and whether medication has a role.
- Regular follow-up
Scheduled appointments to review progress, manage any side effects, and adjust the plan.
- Repeat testing
Laboratory work at defined intervals to confirm things are moving in the right direction and remain safe.
- Maintenance
A specific plan for holding your result, discussed from the beginning rather than at the end.
What Happens During Your Visit
- Consultation
Your provider takes a full history covering your medical conditions, medications and supplements, family history, weight history, previous attempts, sleep, stress, nutrition, and activity.
Your provider will also ask about your relationship with food and any history of disordered eating. This is a routine and important part of the evaluation, because it affects whether certain approaches are appropriate, and it is a conversation held without judgment.
- Testing
Comprehensive laboratory testing is arranged, with instructions on how to prepare. Your provider explains what is being measured and why.
- Results and Planning
You will go through your results together and build the plan around them. Your provider will explain what the testing found, how it relates to your history, and what approach makes sense.
If medication is appropriate, you will discuss it in full, including what to expect, before anything is prescribed. If it is not appropriate, your provider will explain why and what the plan is instead.
- Ongoing Care
Follow-up appointments are scheduled throughout, with repeat testing at intervals. Your provider adjusts based on your progress, your tolerance, and how you are feeling, and coordinates with your other physicians where needed.
You will receive written guidance covering your plan, your follow-up schedule, and when to call. Contact Revive Clinic & IV Therapy if anything concerns you between appointments.
How Much Does Medical Weight Management Cost?
Cost depends on the evaluation and testing required, the plan itself, and the follow-up schedule. Where medication is part of a plan, its cost is separate and will be explained clearly.
Because this is ongoing care rather than a single treatment, Revive Clinic & IV Therapy discusses the full picture during your consultation, including what a first year realistically involves, so you can decide with the whole plan in front of you. You will know what is recommended and what it costs before anything begins, and you are free to decline.
Your first consultation is complimentary for new patients.
First Time Considering Medical Weight Management?
Four things are worth understanding before you start.
Evaluation comes first. Testing may identify a medical contributor that has been making this harder, and knowing that changes the plan. Any program that starts with a prescription rather than an evaluation has skipped the part that matters.
Muscle matters as much as weight. Meaningful weight loss can take lean muscle with it, which affects strength, metabolism, and how you feel. Adequate protein and resistance training are components of the plan here rather than suggestions, and body composition is tracked alongside weight.
Progress is rarely a straight line. There will be weeks where the number does not move. That is normal, it is expected, and it is not a reason to abandon a plan that is working.
Maintenance is part of the plan from day one. Weight regain after stopping is common and can be substantial without a maintenance strategy in place. Your provider will discuss what holding your result actually involves at the beginning, not at the end.
At your consultation your provider will listen to your history, explain what evaluation would tell you, and give you a clear picture of the program and its cost before any commitment.
Maintaining Your Results
Maintenance is where most weight loss efforts come undone, and it is the part that gets the least attention. Revive builds it into the plan from the start.
Your body adapts to weight loss by adjusting appetite signaling and energy expenditure, which is why holding a result requires an active plan rather than simply stopping. Regain after discontinuing treatment is common and can be substantial without one.
What holds results is usually a combination: continued attention to protein and resistance training to protect the muscle you kept, a nutrition approach you can sustain rather than endure, ongoing management of any medical contributors that were identified, and continued clinical follow-up at an interval your provider will set.
Your provider will discuss what maintenance looks like for you specifically, including how long treatment is likely to continue and what happens if you decide to stop.
Why Choose Revive Clinic & IV Therapy?
Revive Clinic & IV Therapy is a nurse-practitioner-led practice. Dr. Gurpreet Bajwa, DNP, FNP-BC, is a board-certified family nurse practitioner who spent more than a decade in urgent care in Ashburn before founding Revive, and who has worked closely with dermatology, endocrinology, and internal medicine. Dr. Bajwa holds a Doctor of Nursing Practice degree and practices under a collaborative practice agreement with a physician, as Virginia requires.
Weight has become a prescription-first business in a lot of places, delivered through a questionnaire with little evaluation and less follow-up. That model moves product. It does not identify the thyroid condition, the medication effect, or the insulin resistance that was making this harder, and it does not help anyone hold the result afterward.
Revive has functional medicine and laboratory testing available in house, which means the evaluation is real and the contributors can be investigated properly. It also means that if your results point somewhere else, your provider can pursue it.
Care here is structured around evaluation, individualized planning, and follow-up, with maintenance discussed from the start.
We are not going to hand you a prescription and a monthly refill. We are going to find out what is making this difficult, treat what we find, and help you hold on to the result.
Enhance Your Experience
Weight management addresses one part of the picture. Patients often combine it with related services.
- Functional medicine and lab testing for thyroid, metabolic, and nutrient contributors
- Hormone optimization where hormonal change is part of what shifted
- IV therapy and vitamin injections where nutritional gaps are identified
- Body contouring for refining specific areas once weight is stable
Each recommendation is made individually during your consultation. Not every service is appropriate for every patient, and results vary. Your provider will advise on sequencing and timing.
Let's Talk About the Result You Want
You know what you are after, and it is probably not a number. Moving more easily. Getting through a day without your body making it harder. Not having this take up so much room in your head. Feeling like your effort produces a result.
Bring that to your consultation, along with an honest account of what you have tried and what happened. The clearer you are about the history, the better your provider can work out what has been getting in the way and what approach is likely to hold.
Your first consultation is complimentary. You will leave knowing what evaluation would tell you, what the program involves, and what it costs, before you commit to anything.
Frequently Asked Questions
What is medical weight management?
It is weight care delivered as clinical care, involving evaluation by a provider, laboratory testing, an individualized plan, and scheduled follow-up. It differs from a commercial program in that it begins by evaluating you and identifying medical contributors before any plan is made.
Do you prescribe weight loss medication?
Whether medication is appropriate, and which option fits, is a clinical decision made after evaluation. For that reason we do not publish a medication list or discuss specific prescriptions beforehand. Your provider will discuss the options that are appropriate for you at your results appointment.
Why do I need lab work first?
Because several medical conditions make weight loss harder and are correctable, including thyroid disease, insulin resistance, hormonal changes, and nutrient deficiencies. Medication effects also contribute. Identifying these changes the plan, and sometimes addressing one of them is a substantial part of the answer.
How quickly will I lose weight?
That varies considerably and progress is rarely linear. Periods where the scale does not move are normal and expected. Your provider will set realistic expectations for your situation rather than a general timeline.
Will I lose muscle?
Meaningful weight loss can take lean muscle with it, which is why adequate protein intake and resistance training are components of the plan rather than optional advice, and why body composition is tracked alongside weight.
What happens if I stop treatment?
Weight regain following discontinuation is common and can be substantial without a maintenance strategy in place. Your body adapts to weight loss by adjusting appetite signaling and energy expenditure. This is why maintenance is discussed from the beginning and built into your plan.
Will you work with my other doctors?
Yes. Coordination with your primary care provider and any specialists is part of the program, particularly where you take other medications or manage other conditions.
Is this covered by insurance?
Revive Clinic & IV Therapy is a fee-for-service practice and does not accept insurance. Where medication is part of a plan, its cost is separate and will be explained clearly before anything is prescribed.
Am I a good candidate?
That depends on your health history, your laboratory results, your medications, and your goals. Your consultation and evaluation exist to answer this question properly, including whether medication has a role for you.
Who is not a candidate?
Certain weight management treatments are not appropriate for individuals with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2, a history of pancreatitis, gallbladder disease, certain gastrointestinal conditions including delayed gastric emptying, or an active or prior eating disorder, or during pregnancy, while planning pregnancy, or while breastfeeding. Diabetic retinopathy affects candidacy in some circumstances. Other medications and medical conditions may also affect what is appropriate. Candidacy is determined only after individual evaluation and laboratory testing by a qualified healthcare provider.
What are the risks?
Medical weight management involves treatments with associated risks, and these depend on the specific approach used. Commonly reported effects include nausea, vomiting, diarrhea, constipation, abdominal pain, decreased appetite, fatigue, and injection site reactions. More serious risks include inflammation of the pancreas, gallbladder disease including gallstones, kidney injury related to dehydration, delayed emptying of the stomach, and low blood sugar, particularly in patients taking other glucose-lowering medications. Some agents in this class have been associated with thyroid C-cell tumors in animal studies. Significant weight loss may also result in loss of lean muscle. Your provider will review the risks specific to your plan in full before anything is prescribed, and ongoing monitoring is a requirement of treatment.
What results can I expect?
Results vary between individuals and depend on starting point, medical conditions, medications, hormonal factors, age, sleep, nutrition, activity, and adherence to the plan. Progress is rarely linear. Weight regain after discontinuation is common without a maintenance strategy. No specific outcome, rate of change, or amount of weight loss can be guaranteed.
Do you accept insurance?
Revive Clinic & IV Therapy is a fee-for-service practice and does not accept insurance.
